The applicant should have the following information ready:

  • Address history for the last five years, including all dates they moved in and out
  • Any former names and the dates they were changed
  • Title, forename, middle name (if applicable) and surname
  • Date of birth
  • Gender
  • Language
  • Telephone number
  • Email address
  • Their birth town, county and country
  • Their birth nationality, and details of any change of nationality with relevant dates
  • Any unspent convictions

 If you haven’t already, you should also familiarise yourself with the ID checking guidelines for DBS applications.

Make sure both you and the applicant are aware of which documents they’ll need to provide to prove their identity, and give the applicant time to gather these documents in advance.

The applicant will need to show you original copies of the documents to verify and support their application. Scanned copies of documents are not acceptable.
